Cook Adaları bayrağı günümüzdeki hâli ile 4 Ağustos 1979 tarihinde göndere çekilerek kullanılmaya başlanmıştır.

Bayrak mavi zemin üzerine oluşturulmuş olup, göndere çekilen bölümün sol üst köşesinde bayrağın dörtte birini kaplayacak şekilde Birleşik Krallık bayrağı bulunmaktadır. Bayrağın dalgalanan tarafında bulunan yarısında ise 15 adet yıldız daire biçiminde konumlandırılmıştır.

Bayrakta bulunan mavi renk, adayı çevreleyen Pasifik Okyanusu'nu sembolize ederken, sol üst köşede konumlanmış Birleşik Krallık bayrağı ise serbest iş birliği içerisinde bulunduğu Yeni Zelanda ve İngiliz Milletler Topluluğu'nu temsilen bulunmaktadır. Beyaz 15 yıldız ise adayı oluşturan on beş adayı temsil etmektedir.

Cook Adaları, 1900 yılında Birleşik Krallık tarafından ilhak edildikten sonra bağlandığı Yeni Zelanda'nın bayrağını 1973 yılına kadar kullanmış,[1] 1973 ile 1979 yılları arasında yeşil zemin üzerine dalgalanan tarafta 15 sarı yıldızı bulunan bayrak kullanmıştır.
