Tirofiban

Tirofiban antitrombosit bir ilaçtır. Glikoprotein IIb/IIIa inhibitörleri adı verilen bir antitrombosit sınıfına aittir. Tirofiban, fibrinojen ile trombosit integrin reseptörü GP IIb/IIIa arasındaki protein-protein etkileşiminin küçük moleküllü bir inhibitörüdür.[1][2]

Jenerik ilaç olarak mevcuttur.[3]

Yan etkiler

Başlıca olumsuz etki, klinik müdahalenin lokal bölgelerinde ve sistemik olarak (vücudun bazı kısımları veya tüm vücut sistemi ile ilgili) kanamadır. Hastaların %1,4'ünde majör kanama, %10,5'inde ise minör kanama meydana geldi. Tüm hastaların %4,0'ında kanamayı sonlandırmak ve kanamaya bağlı anemiyi iyileştirmek için transfüzyon gerekti. Yaşlı hastalar gençlere göre, kadınlar erkeklere göre daha fazla kanama atağı geçirmiştir.

Trombositopeni, tirofiban + heparin grubunda (%1,5) heparin kontrol grubuna (%0,8) göre daha sık görüldü. Bu olumsuz etki genellikle birkaç gün içinde kolayca geri döndürülebilirdi.

Pozitif dışkı ve idrar hemoglobin testleri de rapor edilmiştir.

Pazarlama sonrası olaylar intrakranyal kanama, retroperitoneal kanama, pulmoner kanama ve spinal-epidural hematomun meydana gelmesi olmuştur. Ölümcül kanama nadiren rapor edilmiştir.

Bazen trombositopeni titreme, düşük dereceli ateş veya kanama komplikasyonlarıyla ilişkiliydi.

Anafilaksi dahil aşırı duyarlılık vakaları meydana geldi.

Etkileşimler

Varfarin veya diğer oral antikoagülanların eş zamanlı uygulanması ciddi kanama olayları riskini artırabilir. Tirofiban tedavisi sırasında bu ilaçlarla idame tedavisinin durdurulup durdurulmayacağı kararı sorumlu klinisyen tarafından verilmelidir.
